A support coordinator will collaborate with you to ensure a balanced mix of supports, enhancing your ability to maintain relationships, manage service delivery tasks, live more independently, and participate in your community.
  • Specialist support coordination – This higher level of support coordination is designed for individuals with more complex situations who require specialist assistance. A Specialist Support Coordinator will help you navigate challenges in your support environment and ensure consistent delivery of services.
