At Circle of Hope, we believe that home care in Guildford should be about making you feel comfortable and happy while we attend to your needs. Entering this phase of life might bring its own set of challenges. But we are here to make it easier.
So, if you want someone with whom you can make a genuine connection and who can offer professional, friendly support, Circle of hope services is for you.
But firstly, this might be the first time you are hearing about the HCP Scheme. Here is what the scheme is about.
My Aged Scheme & Home Care Packages (HCP)
The My
Aged Care Scheme is an initiative by the government to help the elderly in Australia by funding their needs, which they cannot afford by themselves, in their old age.
Under this scheme, the government also launched the HCP Scheme. It offers home care Providers Guildford to those senior citizens who want to live independently in their own houses while having someone to help them out with their daily needs.
The good news? Circle of hope services is registered under both of these schemes. So, whether you prefer short-term care, an aged home or at-home care, we can support you in any way.
Other Benefits Of Choosing COH
- Our Professional Team: We have on board highly skilled nurses and management staff to take care of you, from registered and enrolled nurses to care planning and assessment staff.
- Transparent Communication: We will guide you through every step, so you are clear on every single detail. We give you full clarity on what it will be like going forward with our aged care services – no hidden fees or over-the-top charges.
- NDIS-Registered: Not only are we approved under the Aged Care Scheme, but we are also NDIS-registered. So, if you have both aged care and disability requirements, we can make a plan that encompasses both needs.
- Flexible Support: Need home care Providers Guildford for just a few hours a week? Or 24/7? We are flexible with our services and can provide support depending on when you need us.
- Besides Services: Your mental health and social well-being are very important to us. That is why, besides providing services, we help you connect and attend different social gatherings or have some kind of gentle activity like exercise or music.
How We Can Help You
Under the HCP Scheme, there is a wide range of services to cater to the different needs of different people. We have mentioned some of the most common services below. Besides these, we also provide other services that are specific to your needs.
Help with Everyday Activities
- Getting dressed and taking care of personal hygiene
- Showering and toileting
- Cooking meals and dietary care
- Light housekeeping and laundry
- Grocery shopping and errands
Medical & Clinical Care
You may require more specialised care if you have health conditions like dementia, Parkinson’s, or post-surgery recovery. Our team includes nurses and healthcare professionals who can support you with any medical or clinical care.
We also help you remember to take your medication on time.
Transportation
Whether you have a doctor’s appointment to go to or want to visit your friend, we offer
transportation services to ensure that you can safely travel from your house and back.
Home Care Packages
You need to take an assessment to qualify for a home care package. Once you qualify for a home care package, the government allocates a budget to you that covers all or part of your aged care costs. You can also pitch in any additional contributions that the government funding is not covering.
There are four levels of packages, each one with its own set of services that are based on the level of need you require, ranging from basic to complex care.
