The scheme is designed for those with more complex care needs that go beyond what the Commonwealth Home Support Programme can provide.

Am I Eligible For My Aged Care Scheme?

To qualify for assessment, you must meet some needs and age requirements. These check: 

• that you are 65 years or older 
• if you have care needs and are either:

• an Aboriginal or Torres Strait Islander person aged 50 to 64 years
• homeless or at risk of homelessness and aged 50 to 64 years

• what kind of and how much assistance you need. For example, if you have:

• Noticed a change in what you can do or remember 
• Been diagnosed with a medical condition or reduced mobility 
• Experienced a change in family care arrangements 
• Experienced a recent fall or hospital admission

How Can I Receive Government-Subsidised Aged Care Services?

Once, you have checked if you fulfil all the requirements based on the eligibility checker, the next step is to apply for the assessment.  

1. You can apply for an assessment online. This involves a two-part assessment process that understands your needs and what services could help you.
