About Us

COHS is an approved Aged Care Provider for home care services under the Australian Government ‘My Aged Care’ Home Care Packages (HCP) scheme.

Our goal is to expand the organisation through providing a variety of activities and programs targeted to elderly people irrespective of their funding streams.

To ensure COHS’ supports remain best practice and unique, we have a skilled and experienced management team, which involves key people from various expertise, skills, and backgrounds.

Our Services

COHS provides the following supports under My Aged Care:

  • Clinical care including wound care management, diabetes management, complex bowel management and dementia supports.

Care Packages

At COHS, we offer a range of home care packages to suit you based on your needs, goals and preferences. This is so you can get the best care possible. There are four (4) types of home care packages based on your needs, budget and what you’re looking to get out of your care package.

  • Level 1: Basic Care Needs.
  • Level 2: Low-level Care Needs.
  • Level 3: Intermediate Level Care Needs.
  • Level 4: High-level Care Needs.

 

Everyone who qualifies for a home care package is given a budget made up of a contribution from the Australian Government and your own personal contribution. Together, these funds cover your care services as well as the package management costs from your provider to organise and manage your package services.

Level 1

Level 1 home care packages are the most basic package.

It caters to those who have lower and basic care needs. It can also be an option for those who are awaiting a higher level of care to become available.

Level 1 Care packages include assistance with:

  • Domestic or household tasks such as cleaning, dusting, laundry, and dishes.
  • Household maintenance.
  • Gardening services such as lawn-mowing, weeding, and ensuring garden paths are safe, and hazard-free.
  • Transport services.
  • Social activities.


Eligibility

Level 1 home care packages are available to individuals who are:

  • Over the age of 65 (50 years old for Aboriginal and Tores Strait Islander).
  • An Australian resident.
  • Requiring assistance with tasks at home.

Level 2

Level 2 packages are available to those who require a bit more help.

It caters to individuals who have low-level care needs and require support to maintain their independence at home.

Level 2 Care packages include assistance with:

  • Gardening services such as lawn-mowing, weeding, and ensuring garden paths are safe, and hazard-free.
  • Transport services.

  • Over the age of 65 (50 years old for Aboriginal and Tores Strait Islander).

Level 3

Level 3 Home Care Packages provide intermediate-level, day to day home care for its recipients.

Individuals at this level might also qualify for care in an aged care home but prefer to stay in their own homes.

  • Household maintenance.
  • Gardening services such as lawn-mowing, weeding, and ensuring garden paths are safe and hazard-free.
  • Transport services.
  • Meal preparation.
  • Personal care services.

  • Memory and/or behavioural support.
  • Medication management services.

Level 3 home care packages are available to individuals who are:

  • Over the age of 65 (50 years old for Aboriginal and Tores Strait Islander).
  • An Australian resident.
  • Requiring assistance with tasks at home.

  • Gardening services such as lawn-mowing, weeding, and ensuring garden paths are safe and hazard-free.

  • Memory and/or behavioural support.
  • Medication management services.

Level 4 home care packages are available to individuals who are:

  • Over the age of 65 (50 years old for Aboriginal and Tores Strait Islander).

  • Requiring assistance with tasks at home.

Our Team

Our COHS care support team is made up of qualified, skilled, and experienced professionals who are willing to go above and beyond to help you get the most of your package so you can be as comfortable and independent as possible in the safety and enjoyment of your own home.

Our team is made up of, Registered Nurses (RN, Enrolled Nurses (EN), Assistants in Nursing (AIN), Care Support Workers and Care Planning and Assessment staff, who are ready and willing to help you out with all your home and aged care needs.
